This role entails delivering direct care while adhering to designated visit productivity standards.Responsibilities:Manage the holistic case management of home health patients, ensuring high-quality care delivery.Provide direct care to patients while meeting established visit productivity standards.Collaborate closely with the Clinical Manager and interdisciplinary team to optimize patient outcomes.Maintain compliance with industry best practices and evidence-based guidelines in physical therapy.Ensure accurate documentation and meticulous record-keeping of patient care activities.Qualifications:Graduation from an accredited physical therapy program.Certification and licensure as a Physical Therapist (PT).Possession of Basic Life Support (BLS) certification as a Healthcare Provider.Alternatively, consideration will be given to approved license applicants or employees in resident training programs.A minimum of 2 years of recent relevant experience in physical therapy.Our Offer:Our client provides an exceptionally competitive compensation package, encompassing salary, bonuses, relocation assistance, and world-class benefits.
