Wellness Navigator (Swahili)- 2025783

POSITION SUMMARY:

The Wellness Navigator provides individual and group-based wellness services, including essential health literacy instruction, facilitation of wellness groups and individual in-home coaching. The primary goal of this position is to assist refugees and other eligible immigrants to achieve holistic wellness in their new community, including physical and mental health, community connection and family well-being. The Wellness Navigator (Swahili) must be a native speaker of Swahili.

ROLE & RESPONSIBILITIES:

Conduct outreach and enrollment for both the Refugee Health Promotion program and the Services to Older Refugees program. 

Conduct individual client need assessments to identify areas of health navigation and health education that are needed. 

Collaborate with clients to develop and implement individual service plans.
