Transitional Case Manager - Addiction, Substance Use Disorder Service (SUDS) Direct Service - (OPSEU) Full-Time, Permanent (Benefitted) London, Ontario (London Zone) This posting is the result of an existing vacancy - Application Deadline: July 13, 2026 - The Addiction Transitional Case Manager will perform duties and responsibilities consistent with providing a psychosocial model of care to assist in streamlining access to community addiction and mental health supports for individuals who are presenting to hospital services.

This is a front-line, clinical position providing crisis intervention, brief addiction transitional case management, and system navigation related to substance use health.

This position provides support and care pathways to community-based addiction and mental health services in a collaborative manner to reduce avoidable, unnecessary and repeat emergency department visits or admissions.
