The Primary Therapist is responsible for monitoring and delivering high quality services to dually diagnosed/chemically dependent patients. Primary responsibilities include: Coordination of the full continuum of care; Assessment and documentation of clinical needs; Implementing and delivering individual, group and educational services; The provision of a of a safe and therapeutic milieu; Crisis intervention and full participation with the multidisciplinary team toward meeting a patient's treatment plan objectives. Discharge planning and referrals to community resources required.
