Supervisor, Integrated Service Delivery Systems – Role Overview

The Supervisor, Integrated Service Delivery Systems is a unique leadership position that supports Ontario Health Teams’ strategy to improve client transitions through Hospital‑to‑Home and other integrated care programs. The role blends clinical expertise with operational oversight, ensures coordinated scheduling, coaches an integrated care approach within the team and across partners, and maintains ongoing communication with program funders, primary care, Ontario Health atHome (OHAH) and other community services.
