About the Role

Key Responsibilities:

  • Evaluate and diagnose speech, language, cognitive-communication, voice, fluency, and swallowing disorders; assess home environment and identify equipment needs.
  • Develop and implement individualized, physician-approved plans of care with measurable therapy goals.
  • Provide dysphagia therapy and recommend appropriate diet modifications in coordination with the care team.
  • Recommend and train patients and caregivers on AAC devices and home exercise programs.
  • Monitor progress, adjust the care plan as needed, and submit accurate clinical documentation per JCAHO standards.
  • Collaborate with physicians, nurses, PT, OT, and other team members through care conferences and interdisciplinary planning.
