This role serves as a Structural Heart Nurse Navigator: coordinates patient care from the time of referral to the time of implant, and helps navigate them through the referral and screening process. This includes being the initial contact for patients when referred to our team. The Specialty Nurse Navigator will provide patient education regarding the referral, procedural, and post-procedural process.
