Social Worker II - Neuro - Patchogue

We have an exciting opportunity to join our team as a Social Worker II - Neuro - Patchogue.

In this role, the successful candidate will provide psychosocial assessments, diagnosis, and treatment as well as consultation about patients and families to assist them and the health care team in coping with the patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders.
