About the Role

Summary The social worker serves as a member of an interdisciplinary treatment team contributing to overall primary healthcare services to Veterans assigned to a primary care team. The social worker provides clinical counseling, psychosocial assessment, and case management services to eligible veterans and their family members to meet biological, psychological, social, and environmental needs. The social worker adheres to the principles of the Patient Aligned Care Team (PACT) Model.
