Registered Nurse

We are hiring Registered Nurses (RN) or Licensed Practical Nurses (LPN) clinical experience in acute care, ambulatory care, home health, or care management.

Key Responsibilities

• Conduct telephonic and virtual patient assessments
• Educate patients on conditions, medications, and treatment plans
• Develop and manage individualized care plans
• Monitor patient progress and escalate clinical concerns when required
• Coordinate care among providers, payers, and community resources
• Maintain accurate documentation in EHR systems
• Support patient engagement and continuity of care

Required Skill

• Strong clinical assessment and patient communication skills.

• Knowledge of chronic disease management and preventive care.

• Experience with EHR/EMR systems and telehealth platforms.

• Ability to work independently in a remote care environmet
