Summary This Registered Nurse Case Management position is located within Home Based Primary Care (HBPC), Nursing Service at the Thunderbird VA Clinic in Phoenix, AZ. Responsibilities The Registered Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components.
