**Position Purpose**
This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.
**As a Pharmacist, you will:**
+ Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
+ Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
+ Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient...
