Patient Navigator, LVN - Beverly Hills IM Endo Clinic - Full Time 8 Hour Days (Non-Exempt)(Non-Union)

The Patient Navigator is a Licensed Vocational Nurse with recognized leadership abilities and sound clinical skills who will actively assist the physician in the day to day activities and care of the patient. This role will also include arrangement of the details of patient care in the unit's service line. The LVN Patient Navigator takes a leading role in activities in the unit, patient rounding and care, identifying and assessing patient needs, scheduling and working with the Physicians, RN coordinator, employees, and preceptors in the orientation and training of new employees. Working in close collaboration with the RNs and M.D.s, preparing, teaching and guiding patients through the continuum of care.
