Part-time, Monday to Friday, 9:00am - 5:00pm, 15 hours per week.

This is a grant funded position. -Complete patient intake assessments, service plan development, reassessments and service plan updates as scheduled for pts' as assigned by the program. -Conduct out-reach and in-reach activities. -Facilitate supportive individual. (and group as needed) counseling sessions for a small caseload assigned by the program coordinator. -Remind pts' of medical appt's, conduct missed appt. follow up to reengage and assist with linkage to care. -Assist patients' in completing f...
