We are seeking an enthusiastic and motivated Nursing Associate or newly qualified Nursing Associate to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, Advanced Nurse Practitioners, and Care Coordinators, supporting the delivery of high-quality, patient‑centred care across our member practices. You may have just completed your trainee nurse associate training or already be a qualified nurse associate.

Main duties of the job

The successful candidate will play a key role in managing and supporting our frailty cohort, including palliative care patients, house‑bound individuals, and those living with frailty.
