Nurse Practitioner (RFT 1.0 FTE)

Duties

• Support triage, assessment, and follow‑up care for individuals presenting in crisis.
• Works in partnership with the program psychiatrist to deliver short‑term treatment plans for clients who meet appropriate criteria, working independently with appropriate oversight from the psychiatrist.
• Integrates clinical findings and evidence‑based practice to formulate diagnoses and develop initial care plans.
• Collaborates with patients and members of the interdisciplinary team to identify and implement pharmacological and non‑pharmacological interventions that promote optimal outcomes.
• Participate in the dissemination and utilization of best practice guidelines.
• Provide education, mentorship and support to the health care team, primary care providers, hospital partners, and community groups.
