About the Opportunity

The Nurse Practitioner (NP) will provide primary health care across a designated group of long‑term care homes. This role supports care through collaboration with interdisciplinary teams, physicians, staff, residents, and families to enhance quality of care. The successful candidate will primarily support, North Queens Nursing Home, Rosedale Nursing Home, Hillside Pines and Ryan Hall and may provide coverage to additional homes based on operational needs.

The Nurse Practitioner provides comprehensive clinical care to patients in a designated service or program areas.
