Job Summary :
Licensed Practical Nurse (LPN) must abide by the CMS Conflict of Interest Policy. Come to work (in person/onsite) as scheduled and consistently demonstrate dependability and punctuality. The incumbent must consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, Persons served, families and consultants. Additionally, the incumbent is required to always present a professional appearance, accept assigned duties in cooperative manner from DON & QIDP, and assume accountability for data contained in the employee' s handbook. This position may require working some weekends.

Job Description :
- Read nurse's log daily.
- Monitor Individuals daily for any change in health status.
- Administer medication at designated times.
- Daily documentation on MARS.
- Check adaptive equipment weekly (helmets, walkers, gait belts, wheelchairs etc.) <...
