- Bachelor's, Master's, or D.P.T. degree from an accredited Physical Therapy Program.
- Oregon Physical Therapist License upon hire.
- Physical Therapist License in state of hire and wherever care is delivered.
- National Provider BLS certification from American Heart Association upon hire.
Additional Requirements:
- Specialized physical therapy training or experience may be required occasionally to meet department needs.
