An exciting opportunity has arisen for an experienced and motivated Complex Case Manager to join our Continuing Healthcare (CHC) Team. This rewarding role supports adults with complex health needs requiring coordinated, person-centred care and support. As a Complex Case Manager, you will ensure individuals receive the right care, at the right time, in the most appropriate setting, achieving the best possible outcomes for patients, families and carers. Working across health and social care systems, you will undertake assessments, coordinate complex care packages and work collaboratively with stakeholders to deliver high-quality Continuing Healthcare services.
