Philadelphia post acute care facility is seeking a Nurse Practitioner or Physician Assistant to join their team. This is a full time opportunity rounding in skilled nursing facilities.
Perform comprehensive patient assessments, including medical histories and physical exams. Develop, implement, and adjust individualized care plans based on patient needs. Monitor progress and coordinate care to optimize outcomes. Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach. Educate patients and families on health management and preventive strategies. Maintain accurate, timely medical records in compliance with regulations. Estimated compensation range: $127K-$151K.
