Home Care Nurse

Position Description

Reporting to the Nursing Manager, the KCA Home Care Nurse will provide Home Care Nursing services in the KCA member communities. The Home Care Nurse will work in partnership with other members of the health care team to provide holistic care to the entire community population. The Home Care Nurse will provide assessments, screening, healthy lifestyle support, education and chronic disease management, home care, immunization and communicable disease programming with an ultimate goal of improving health outcomes and facilitating access to services.

Term: Full time Permanent

Salary: 65,000 to 79,000 based on experience

Location: 240 Veterans Drive, 3rd Floor, Kenora, Ontario
