Salary Range: $20.17 To $21.25 Annually
$1200 sign on bonus
Unity House of Troy, Inc., a non-profit multi services agency dedicated to enhancing the lives of people in need has an opening for a Social Determinants of Health Liaison in our Supported Housing program for those living with HIV/AIDS.
This position has the option of a hybrid schedule.
Responsibilities:
Refer and link clients to supportive services that improve medical outcomes.
Assist participants to access emergency financial assistance, transitional housing and other resources.
Complete assessments, support plans, and other required program documentation.
Improve access to care though referral, linkages and care coordination.
Collaborate and act as a liaison with other services providers in the community.
