Working at QEII /Mackenzie Place the HCA has an exciting opportunity to provide care for clients who are medically stable and could be transitioning from the hospital to home, or from one care setting to another. Reporting to the Care Manager, the HCA will assist clients with personal health care needs to promote and maintain their ADLs at Mackenzie Place. The incumbent will help support a diverse clientele which may include transitions from hospital to home, or from one care setting to another. The HCA is expected to work with the health care team using a team focused approach in managing workloads while adhering to the client care plan. The HCA will work collaboratively with the health care team to promote and maintain a holistic approach in client care.
