EAST:MK Primary Care Network has exciting opportunity to welcome a general medical practitioner into our practice teams. The role is offered on sessional basis of between 4 to 6 sessions per week, between Monday and Saturday, on a rotation basis across the 4 surgeries that make up the PCN: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery.

Main duties of the job
- Deliver high quality clinical care to patients through face-to-face/online/phone consultations, home visits, assessments, diagnoses, treatment plans. Manage acute & chronic conditions, safe prescribing, referrals & prescription reviews, maintain accurate clinical records
- Deliver care plans for chronic conditions
- Promote immunisations, screenings & lifestyle interventions. Focus on early intervention, prevention & education
