Coordinator, Discharge Planner

JOB SUMMARY:

• Under direction oversees the resident discharge process for the Sub-Acute Unit and performs the work involved in providing needed social assistance.

ESSENTIAL JOB FUNCTIONS:

• Participates and takes the lead in the utilization review process for the Sub-Acute Unit and prepares residents for discharge.


• Performs insurance reviews and issues the cut letters for Medicare clients.


• Participates with the staff relevant to diagnosis and the design of effective treatment plans.


• Provides social services to assist residents in the adjustment to their environment.


• Reviews cases of admissions and interviews patients to assure that certifications are in accordance with hospital criteria.
