An exciting opportunity has arisen to join the Rehabilitation and Frailty Team (RAFT). We are looking for an experienced and motivated health care worker who is passionate about providing a high standard of patient‑centred care. You will work closely with community nurses, community matrons/care coordinators, physiotherapists, occupational therapists and rehabilitation assistants who are committed to maintaining independence, promoting self‑care and assisting patients to achieve their goals.

The successful candidate will be hardworking, reliable and able to use initiative and work flexibly to respond to the service needs. You will be part of a friendly and approachable team, and will be well supported by your colleagues and line manager.
