Job Description of VBC Clinical Programme Executive
ERAS Implementation and Care Coordination (45%)- Implement and monitor ERAS protocols across designated surgical conditions
- Coordinate pre-operative assessments and patient education sessions and materials
- Provide pre-operative counselling and education to patients and families
- Provide post-operative discharge support services to patients within 30 days of discharge
- Monitor protocol compliance and document variances
- Coordinate care across multidisciplinary teams throughout the patient journey
- Track and follow up on patient outcomes and recovery metrics
- Facilitate communication amongst various healthcare providers
