Job Summary The In-Home Client Care Coordinator oversees individualized care plans for clients receiving services in their homes.

This role ensures clients receive high-quality, person-centered care while supporting caregivers, collaborating with healthcare providers, and maintaining compliance with agency, state, and federal guidelines.
