Primary Job Title: Case Management RN

Location: Lodi, CA

Onsite Flexibility: Onsite

Contract Details:

• Position Type: Contract
• Shift / Schedule: Day Shift, 8:00 AM 6:30 PM, Monday Friday with variable weekends, 40 hours per week, no call
• Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Pay Rate: $65.00 / Hour (USD)

Key Responsibilities:

• Manage patient cases and coordinate care across healthcare settings
• Utilize Cerner electronic health record system for documentation and clinical decision support
• Develop and implement care plans based on acute care assessments

Education Requirements:

• Registered Nurse (RN) licensure in the state of practice: Required
• Bachelor's Degree in Nursing (BSN): Preferred
