Case Coordinator NISS. - Permanent Full-time - Parramatta

About the Role

Employment Type: Max-term 6 months, Full Time, 38 hours per week.

Office Location: Parramatta, with flexibility to work from office and home.

As a Case Coordinator, you will provide personalised and responsive case coordination services to clients.  In this role you will;

• Conduct thorough assessments to determine client eligibility


• Work with clients to create and regularly update their personalised case plans,


• Assist clients with referrals to appropriate community supports and resources


• Ensure client safety by performing risk assessments, creating safety plans and accurate record keeping


• Build strong relationships with clients and their families, using strengths-based and culturally sensitive approaches
