Care Navigator - Transfer of Care Hub

About the Role:

We are looking to recruit a motivated and compassionate Care Navigator to join our Transfer of Care Hub within Adult Social Care at Wirral Council.

This is a key role supporting safe and timely discharges from hospital, working closely with health and social care professionals to ensure individuals receive the right support at the right time. You will help people transition from hospital settings back into the community, promoting independence and wellbeing.

About the Candidate:

We are looking for someone who has experience working in a health or social care environment and has a strong understanding of adult social care legislation, including the Care Act and Mental Capacity Act.
