Advanced Clinical Practitioner - UEC

An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent and Emergency Care (UEC) Services which include Urgent Community Response (UCR), Virtual Ward (VW) and Integrated front door (IFD).

The post will form part of a seven-day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health, a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.

We are looking for Advanced Clinical Practitioner's (ACP) who share our passion for providing excellent quality of care and can devise and support the delivery of a holistic and thorough treatment plan to service users in their own home. The post holders will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.
