This Advanced Clinical Practitioner role will involveworking alongside our multi-disciplinary team which consists of a GP Partners,Advanced Clinical Practitioner Partner and Pharmacist Partner. They are allsupported by Advanced Clinical Practitioners who lead on acute same day care,alongside the Home Visiting Team, First Contact Physio, Pharmacists,Practice Nurses and HCAs. We also have Social Prescribers who work as PCNstaff dealing with the Mental Health of our patients.
