**Job Summary and Qualifications**
**The Advanced Clinical Pharmacist is responsible for providing safe, effective, evidence-based, and cost-effective medication therapy management for acute care patients. They apply advanced knowledge, skills, and appropriate clinical judgment in all aspects of their work. They serve as a patient advocate when providing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner.
